Dear ChoicePoint Customer: As a part of ChoicePoint's settlement with the Federal Trade Commission (FTC), on March 1, 2006, we began faxing and e-mailing to over 60,000 agency owners and principals the "Customer FCRA Permissible Purpose Certification" forms for signature. This is a written certification stating the nature of the customer's business and the purposes for which the customer intends to obtain consumer reports. To date, the response rate has been outstanding. As of March 30, 2006, we have collected over 33,000 signed agent certifications. However, approximately 27,000 forms remain outstanding. As you know, the completion of this form is mandatory. If the certifications are not signed and returned by June 15, 2006, the non-responsive agencies will be unable to order consumer reports from ChoicePoint. In order to avoid any interruptions in your business processes, we request your assistance in communicating the importance of completing and returning this form. Therefore, we have attached a document with language that you may post on your Web site or communicate to your sponsored agents. Click here to access this document. As we move forward through this collection process, we also want to keep you updated on our progress. Each month your Company receives a list of agencies with an active sponsored account for ordering ChoicePoint products on behalf of your company. Starting with the April 17th release, this report will have an additional column labeled "Certification Signed". Click here to access a sample of this report. The field has two values: Y = Agency has
signed and returned the certificate. The updated spreadsheet will also contain information you have received on previous reports. Please use this report and the attached document as communication tools for your Marketing Representatives. This information will identify sponsored agents in their territories who have not completed the FTC form.
